Application requirements

To finalize your application, please provide the following documents:

1. Company Formation Document
   This is the document filed with the Secretary of State in the U.S. state where your business is registered.
2. IRS-Issued EIN Document
   A document issued by the IRS that includes your Employer Identification Number (EIN).
3. Government-Issued ID: Identification is required for each founder or majority owner of the company. ID documents must include your photo, date of birth, issue and expiration dates, and identification number.

   Accepted Forms of Identification:

   • U.S. Citizens: Driver’s License, State ID, Passport, or Passport Card.
   • U.S. Permanent Residents or Non-Citizens: Permanent Residency Card or Employment Authorization Card.
   • International Applicants: Passport.
